Three core services

One care system, three services by your side

Visits, chronic condition management and medication reviews all happen within one care system, so nothing falls through the cracks.

01

Chronic Care Management

Steady care, peace of mind every day.

Living with a chronic condition is more than taking medicine every day. It means watching your blood pressure, remembering follow-up visits and wondering whether all your medications are safe together. Chronic Care Management gives you a care team of a physician, a pharmacist and a care coordinator who work together to look after you every month.

ForPeople living with two or more long-term conditions, such as high blood pressure, diabetes, heart disease, COPD or arthritis, that need regular monitoring.
What you get
A physician sets up and adjusts a care plan that fits your health.
A pharmacist manages all your prescriptions and checks for interactions when needed.
A care coordinator calls every month to check your readings and new symptoms, review your medications and remind you of appointments.
Any unusual changes are flagged to your physician right away.

3

Follow your care plan and reminders. If something changes, your physician is informed right away.

02

Medication Management & Review
